SMS Terms

Texts are opt-in only. We only message you if you tick the SMS box on our form. Consent is not a condition of using IntakeFormHealth, and you can stop the messages at any time by replying STOP.

Program description

IntakeFormHealth sends text messages to people who have asked for early access and opted in. Messages relate to your own request and nothing else:

We do not send marketing or promotional texts, and we do not text you about anything other than the request you submitted.

Message frequency

Message frequency varies with your own activity. Most people receive a handful of messages around a single scheduled call. We do not send recurring campaigns.

Cost

Message and data rates may apply. We do not charge for text messages; your mobile carrier may, according to your plan.

How to stop

Reply STOP to any message and we will stop texting that number. You will get one confirmation that the opt-out worked, and nothing after that. Opting out of texts does not remove you from early access — we will simply reach you by phone or email instead.

Carriers

Carriers are not liable for delayed or undelivered messages. Delivery is not guaranteed, and we do not use text messages for anything time-critical.

Your number is never sold or shared

Mobile phone numbers and SMS opt-in consent are never sold, rented, or shared with third parties or affiliates for their marketing purposes. We share your number only with the messaging provider that delivers the texts on our behalf, and only for that purpose. See our Privacy Policy for the full picture.

Health information

We do not put medical detail in text messages. A text may say that your profile is ready; it will not contain diagnoses, medications, or anything clinical. Please do not text medical information to us either — SMS is not a protected channel.

Changes

We will update this page if what we send changes, and move the "last updated" date at the top.
